The Office of the Registrar plays an important role as one of the school’s frontline offices, serving students and other clients through efficient and reliable academic records services. An effective Registrar’s Office requires competent personnel and staff committed to providing quality service in matters related to admission, registration, graduation, and student records.

  • Implement policies and procedures relative to the admission, registration, and graduation of students.
  • Serve as recorder and keeper of student records.
  • Promote a more friendly customer relationship with clientele.

Services

Functions of the Office

  • Provides services related to students’ admission and graduation.
  • Promptly and correctly complies with requests for scholastic records and other papers for reference and other purposes.
  • Provides communication and records services to administrators, faculty and staff, students, and other clients.
  • Accurately prepares and submits reports required by the Department of Education (DepEd), Commission on Higher Education (CHED), and other agencies on standard forms in consultation with and with the approval of the School Head.
  • Prepares other reports according to the nature and need that require their attention.
  • Renders consultative service to school personnel and other clients in matters related to the office.
  • Safeguards the confidentiality of all records and materials found and processed in the office.

Requests Handled

The Office of the Registrar processes requests for the following documents and certifications:

  1. Official Transcript of Record (OTR)
  2. Diploma
  3. PRC
  4. Graduation
  5. Medium of Instruction
  6. Translation of Diploma
  7. Grades and Units Earned
  8. Officially Enrolled

Request Workflow

The request process generally follows these steps:

  1. Fill out the request form at the Registrar’s Office counter.
  2. Submit any lacking requirements, if needed.
  3. Pay the certification, OTR, or diploma fee at the Cashier’s Office.
  4. Present the receipt to the staff at the Registrar’s Office and wait for the announcement on when the request will be released.
  5. Receive the requested certification, OTR, or diploma and sign the logbook upon claiming the document.

The office workflow includes assistance to the client, verification of records, checking of requirements, payment confirmation, processing and signing of documents, and release of the requested documents.
